Vivitrol is a long-acting, once-monthly injection of naltrexone that is used as part of a comprehensive recovery plan. It is FDA indicated to help treat alcohol dependence and to help prevent relapse to opioid dependence after a person has completed opioid detoxification. Vivitrol is not a standalone cure, and it works best when paired with counseling, therapy, and recovery supports.

What Is Vivitrol

Vivitrol is an extended release injectable form of naltrexone that lasts approximately one month per dose. Naltrexone is not an opioid and is not addictive. It works by blocking opioid receptors in the brain, which can reduce the rewarding effects of alcohol and block the euphoric effects of opioids. This mechanism can help support sobriety and reduce relapse risk in appropriate patients.

Because it is long acting, Vivitrol removes the need for daily medication decisions. This can help patients remain more consistent with treatment, particularly during periods of stress, low motivation, or disrupted routines.

What to Expect During a Vivitrol Appointment

A typical Vivitrol appointment includes a brief check in on recovery goals and current symptoms, review of side effects or challenges, confirmation of safety requirements including opioid free status when applicable, administration of the injection by a trained provider, and clear follow up planning.

Vivitrol is administered as a deep intramuscular injection in the gluteal muscle by a healthcare professional. Most patients find the visit straightforward and often easier than anticipated.

Important Safety Considerations

Vivitrol is not appropriate for everyone. Your provider will review your medical and substance use history carefully to determine whether it is safe and appropriate for you.

If Vivitrol is used for opioid relapse prevention, you must be fully detoxed and opioid free before starting. Beginning Vivitrol too soon after opioid use can trigger sudden and severe withdrawal. Clinical history and or testing may be used to confirm safety prior to the first injection.

Because Vivitrol blocks opioid effects, attempting to override this blockade by using large amounts of opioids can be extremely dangerous and may result in fatal overdose. Additionally, after Vivitrol wears off or if a dose is missed, opioid tolerance may be reduced, increasing overdose risk if opioids are resumed. Overdose education and safety planning, including discussion of naloxone, are strongly encouraged when appropriate.

Injection site reactions such as soreness, swelling, or irritation can occur. Rarely, reactions may be severe and require medical evaluation. We review what is expected and what is not, and we encourage prompt contact if concerning symptoms arise.

Naltrexone can affect liver function in some individuals. Medical history and laboratory testing may be reviewed based on individual risk factors.

Mental health symptoms such as mood changes, anxiety, sleep disturbance, or depression can occur during recovery. We monitor mental health closely and adjust the overall treatment plan as needed.
