New patients, please READ 

• Your first visit is very important to us and thus we do need to gather various health and other relevant information from you (the patient)
• Please make sure that you complete all your intake forms and consents prior to your visit, the provider will not be able to see you without consents. 
• If you have any prior testing (blood work, neuropsychological testing) done before, it is always appreciated to bring to your appointment and share with your provider. 
• Please be mindful that despite your visit being telehealth, it is crucial that you are in a place where you have privacy, you are safe, and feel comfortable discussing sensitive topics. 
• The patient must be in Colorado during all visits. The provider will NOT be able to see the patient if they are out of state. 
• Minors younger than 15 years old, must be accompanied by their legal guardian or parent. 
• During your first visit you will be asked to discuss childhood history and many other events leading up to where you are today. We understand to some, this might be a sensitive topic, but it is very important for the provider to learn everything about you including your childhood history. 
• Once your appointment is confirmed, intake forms and consents will be emailed for you to fill out and sign. You may also find them in our site.
